Terms and Conditions for Comprehensive Wellness Package

1. General

1.1 These terms and conditions ("Terms") apply to all participants of the Comprehensive Wellness Package ("Package") offered by Karv ("Company") together with Dr.Mubeen.
1.2 By purchasing and participating in the Package, you ("Participant") agree to be bound by these Terms.

2. Package Components

2.1 The Package includes the following components: Evolt 360 Body Scanner, Personalized Blood Work and a Consultation with Dr. Mubeen.

3. Appointments

3.1 Initial consultations and follow-up appointments with Dr. Mubeen will be scheduled based on availability.
3.2 Blood tests will be scheduled at the Grand Harbour Medical clinic following the initial consultation based on availability and the discretion of Grand Harbour Medical.

4. Payment and Insurance

4.1 The initial consultation with Dr. Mubeen at Karv is complimentary.
4.2 Subsequent blood tests and follow-up consultations at Grand Harbour Medical are dependent on the Participant's health insurance coverage. Participants are responsible for any costs not covered by their insurance.
4.3 Payments for services not covered by insurance must be made in full at the time of the appointment.

4.4 The Evolt 360 body scan done at Karv Gym is $CI20 for non Gym Members and $CI10 for Karv Gym Members.

5. Cancellation and Rescheduling

5.1 Appointments can be rescheduled up to 24 hours before the scheduled time without penalty.

6. Confidentiality

6.1 All personal health information collected during the Package will be kept confidential in accordance with applicable privacy laws.

6.2 Participants agree that Dr. Mubeen may share relevant health information, including blood work results, with Karv and the assigned personal trainer for the purpose of optimizing the Participant’s fitness and health plan, unless the Participant expressly requests otherwise.
6.3 The Company will only share Participant information with authorized personnel and medical professionals involved in the Participant's care.

7. Liability

7.1 The Company and its affiliates, including Dr. Mubeen, shall not be liable for any direct, indirect, incidental, or consequential damages arising out of the Participant's use of the Package.
7.2 Participants assume full responsibility for consulting with their primary healthcare provider before starting the Package, especially if they have pre-existing medical conditions.

7.3 The Evolt 360 Body Scanner is not suitable for pregnant women. Pregnant participants are still eligible for the Personalized Blood Work and Consultation components of the Package.

8. Use of Information

8.1 Participants agree that the Company may use anonymized data collected through the Evolt 360 Body Scanner and blood work for research and marketing purposes.
8.2 Any use of Participant testimonials or images will require prior written consent.

9. Modification of Terms

9.1 The Company reserves the right to modify these Terms at any time and it will be uploaded to the website.
9.2 Continued participation in the Package after any modifications constitutes acceptance of the new Terms.

10. Governing Law

10.1 These Terms shall be governed by and construed in accordance with the laws of Cayman Islands.
